At TogetherNest Ltd, our supported living service empowers adults with additional needs to live independently while receiving the day-to-day support they require. Whether living alone or sharing accommodation, we work alongside individuals, families, and local authorities to deliver flexible, person-centred care.

Who We Support

Our supported living service is ideal for adults aged 18+ who:

What Support Looks Like

Our support is shaped by detailed assessments, personal goals, and agreed care plans. Depending on need, support can be offered from a few hours a week to several hours per day.

Support may include:

> Daily routines and personal organisation

> Cooking, shopping, and meal planning

> Managing finances, benefits, and tenancy

> Building relationships and accessing the community

> Attending medical appointments or therapies

> Developing confidence, routines, and life skills

> Emotional wellbeing and behavioural support

> Risk management and safeguarding

Flexible, Responsive Care

Daily scheduled visits

Weekly progress reviews

Care plans are reviewed monthly or sooner based on changes in need or risk.

Accommodation Model

We support individuals in:

Rented flats or houses with visiting support

Shared homes with private bedrooms and communal areas

Single-occupancy units funded by local authorities or housing associations

Note: We do not own properties but work closely with trusted housing partners to match tenants with the right setting.
