Supported Accommodation (16–17-Year-Olds) – TogetherNest Ltd

A safe place to grow, heal, and prepare for independence. At TogetherNest Ltd, our Supported Accommodation service is designed to meet the complex needs of young people aged 16–17 who require a safe, stable environment as they transition into independent living.
We provide not just a roof over their heads, but holistic, wraparound support that builds confidence, skills, and emotional resilience.

We know that many of the young people we support may have experienced trauma, instability, or a lack of consistent care—and our approach is built around trust, structure, and respect.

Who We Support

Our service is ideal for:

Young people leaving foster care or residential care
Unaccompanied asylum-seeking children (UASC)
Those at risk of homelessness or family breakdown
Young people under youth offending services
Young people needing structured, 24/7 support while preparing for full independence
Each young person receives a placement plan aligned with local authority requirements and their individual needs.

What We Provide

TogetherNest offers a range of accommodation-based support services, tailored to each placement and pathway plan. Homes are located in safe, accessible neighbourhoods and are designed to offer both privacy and connection.

Support Includes:

> Structured daily routines and independent living skills.

> Support with budgeting, cooking, cleaning, and appointments.

> Help accessing education, training, and employment.

> Mentorship and emotional wellbeing support.

> Behaviour management and de-escalation.

> One-to-one keywork sessions with monthly reports.

> 24/7 on-call emergency support.

> Safeguarding assessments and incident reporting.

> Cultural sensitivity and language support where needed.

> We support young people in shared or solo placements, based on risk assessments and referral goals.

Our Accommodation

We operate a network of fully furnished, Ofsted-ready properties with:

Private bedrooms for each young person

Shared kitchens and living spaces (or solo flats where appropriate)

Clean, maintained communal areas

Fire safety systems and CCTV in communal areas

Wi-Fi and basic amenities

All properties meet local standards for young person safety, security, and habitability.

Staffing and Oversight.
Each placement is supported by:

> A dedicated key worker

> Rotating staff team with sleep-in or waking night cover (where required)

> 24/7 phone contact line

> Monthly supervision, placement reviews, and reports sent to social workers

> All staff are DBS-cleared, trained in trauma-informed practice, safeguarding, youth engagement, and crisis response.

Personal Progression Planning

Each young person is supported with:

> A personalised pathway plan with SMART goals

> Independent living assessment checklists

> Regular review meetings with the young person and local authority

> Referrals to local education, apprenticeships, and health providers

> Emotional and behavioural goal setting

> We focus not just on where the young person is now—but where they want to go.
